Course Introduction

Incidents and accidents in the workplace can result in serious injuries, operational disruption, financial loss, and reputational damage. Effective investigation techniques are essential to identify root causes, prevent recurrence, and improve overall safety performance.

This course provides participants with systematic methodologies, analytical tools, and practical skills required to conduct professional incident and accident investigations. It emphasizes root cause analysis, evidence collection, interviewing techniques, reporting standards, and the development of corrective and preventive actions.

2.     Types of evidence (physical, documentary, digital, human)

3.     Documentation techniques (photography, sketches, logs)

4.     Chain of custody and integrity of evidence

Day 3: Root Cause Analysis Techniques

1.     Immediate causes vs. root causes

2.     Root Cause Analysis tools (5 Whys, Fishbone Diagram, Fault Tree Analysis)

3.     Human error and organizational factors

4.     Identifying systemic weaknesses and control failures

Day 4: Interviewing and Reporting Techniques

1.     Effective interviewing strategies and communication skills

2.     Behavioral considerations and avoiding bias

3.     Structuring a professional investigation report

4.     Writing clear findings, conclusions, and recommendations

Day 5: Corrective Actions and Prevention Strategies

1.     Developing corrective and preventive action plans

2.     Risk assessment and hazard control measures

3.     Monitoring effectiveness of corrective actions

4.     Lessons learned, continuous improvement, and safety culture enhancement
